Self-Improvement Questionnaires

These questionnaires are provided to help people better understand themselves. They are not psychological tests, medical examinations or mental health professionals examinations. They are self-administered questionnaires that help people understand many of their problems, concerns or interests and thereby reduce many of their worries, fears and anxieties. In some instances questionnaire results may help provide direction or guidance to medical mental health resources. These questionnaires are not intended, nor should they delay, defer or replace medical or mental health professionals examinations and recommendations. That said, Self-Awareness questionnaires are listed and described for your review.

QUESTIONNAIRE DESCRIPTION
Self Esteem Questionnaire ......... Measures a person's perception of self. Self-esteem incorporates an attitude of acceptance-approval versus rejection-disapproval of oneself. This questionnaire's report describes the person one believes oneself to be.
Loneliness Questionnaire ........... Everyone has experienced loneliness, but chronic and severe loneliness is a very painful and demoralizing experience. The more you understand about yourself the more effectively you can deal with loneliness. Loneliness is more a state of mind than close proximity to others.
Anger Questionnaire ................... Anger is a normal human feeling. Yet, when anger is not expressed appropriately there can be serious negative consequences. The Anger Questionnaire assesses your anger so you can understand it better. Should you have anger-related problems appropriate interventions and treatments are recommended.
Alcohol Questionnaire ................ Measures alcohol use and the severity of abuse. Alcohol refers to beer, wine and other liquors. Identifies problematic drinking and screens serious abuse.
Drugs Questionnaire .................. Most people are aware of the consequences associated with others knowing about a person's drug use. So where can somebody get an objective and confidential assessment of their drug involvement? The Drugs Questionnaire is an option.
Depression Questionnaire ......... Depression analysis varies directly with self-reported symptoms and concerns. This questionnaire measures dejected or self-depreciating emotional states that vary from normal to pathological.
Anxiety Questionnaire ............... Identifies unrealistic and excessive worry. When a person is anxious there are many symptoms including motor tension, hyperactivity, vigilance and constant monitoring of surroundings that must be screened before a clear profile emerges.
Gambler Questionnaire ............... With widespread gambling, how can a person know when they have a gambling problem? Is gambling addictive? Do I have a pathological gambling problem? These are questions the Gambling Questionnaire addresses.
Internet Addiction Questionnaire Everybody under 90 years of age is involved with the internet. How can we distinguish between heavy involvement and addictive or pathological involvement? The Internet Addiction Questionnaire gives you an objective and confidential opinion.
Substance Abuse Questionnaire The Substance (alcohol and other drugs) Abuse Questionnaire pertains to alcohol and/or drugs. Alcohol is a licit substance. "Drugs" incorporates both prescribed and non-prescribed drugs. A proprietary substance abuse screen is used with reformatted DSM-IV criteria to identify, and when present, measure the severity of substance use.
Distress Questionnaire ............... Distress has been recognized for thousands of years, e.g., the Book of Job. In the Distress Questionnaire the term "distress" is used generically. Common signs of distress include fear, stress, apprehension, headaches, anxiety, depression, social withdrawal, pain, fatigue, etc. This questionnaire screens distress and related symptomotology.
Stress Coping Abilities ............... Everybody experiences stress but some people handle it better than others. This questionnaire measures how well you handle tension, pressure and stress. It goes beyond simply determining if you have stress. Unmanaged stress exacerbates emotional and mental health problems. How well do you manage or cope with the stress in your life?
Barriers to Employment ............. Many unemployed people want to work but have personal, emotional and attitudinal problems that have become barriers to employment. Before these barriers can be corrected they must be understood. Only then can these barriers be overcome.
Post Traumatic Stress ................ The important feature of Posttraumatic Stress Disorder is development of avoidance symptoms following exposure to an extremely traumatic event. You could have experienced or witnessed the traumatic event or the event could also involve threat of serious harm, death or injury to a family member or close friend. Find out if your fears are PostTraumatic Stress related.
Alzheimer's Questionnaire ......... Alzheimer's is characterized by the development of multiple cognitive deficits (including memory impairment). Until other dementias are ruled out Dementia or Cognitive Disorder may apply. This Questionnaire is an Alzheimer's Screen to be completed by a loved one, caring relative or caregiver.

All Self-Improvement Questionnaires are alphabetically listed. Once a Questionnaire is selected it can not be substituted or changed. Upon completion, your Questionnaire report will appear in a window on your monitor. From that window you can review your report, print your report, e-mail it or save it on your computer. The choice is yours. In other words when your report appears on your computer screen you can do one of 4 things: print your report, e-mail your report, save your report or delete your report by closing the window. However, once you close this window your report is deleted from Self-Improvement memory and can not be reprinted. This procedure protects your privacy and your confidentiality.

As new Self-Improvement Questionnaires are made available to the public they shall be included in this list of available Questionnaires.
